Bernie Sanders (Vermont\u2013Ind.), has quickly become a key issue for progressive voters evaluating Democratic Party candidates for the 2018 midterm elections and the 2020 presidential race. The plan would provide coverage for the <a href=\"https:\/\/www.reuters.com\/article\/us-usa-healthcare-uninsured\/u-s-healthcare-uninsured-rises-most-in-near-decade-gallup-idUSKBN1F523O\">40 million currently uninsured<\/a> in the United States, a gap that is estimated to cause <a href=\"http:\/\/annals.org\/aim\/fullarticle\/2635326\/relationship-health-insurance-mortality-lack-insurance-deadly\">tens of thousands of deaths<\/a> annually. Despite this, Medicare for All has <a href=\"https:\/\/www.nytimes.com\/2017\/10\/24\/opinion\/medicare-for-all-democrats.html\">received<\/a> no <a href=\"https:\/\/www.forbes.com\/sites\/sallypipes\/2018\/07\/09\/choking-on-the-cost-of-medicare-for-all\/#8e6565c56f3c\">shortage<\/a> of <a href=\"https:\/\/www.npr.org\/2016\/05\/09\/477402982\/study-sanders-proposals-would-add-18-trillion-to-debt-over-10-years\">negative<\/a> <a href=\"http:\/\/thehill.com\/opinion\/healthcare\/399283-medicare-for-all-is-a-socialists-dream-and-an-american-nightmare\">coverage<\/a> in the <a href=\"https:\/\/www.usatoday.com\/story\/opinion\/2017\/09\/20\/bernie-sanders-medicare-for-all-plan-all-wrong-for-america-marc-siegel-column\/679134001\/\">media<\/a>, all revolving around the same question: Just how are we going to pay for it?<\/p>\n<p>A <a href=\"https:\/\/www.mercatus.org\/publications\/federal-fiscal-policy\/costs-national-single-payer-healthcare-system?utm_source=bridge&amp;utm_medium=bridgepost&amp;utm_campaign=medicareforall\">study<\/a> on the cost of Medicare for All was recently conducted by Charles Blahous for the libertarian-leaning Mercatus Center at George Mason University. Could Cost $32.6 Trillion, George Mason Study Says\u201d (<b>Time<\/b>, <a href=\"http:\/\/time.com\/5352950\/medicare-trillions-bernie-sanders\/\">7\/30\/18<\/a>)<\/li>\n<\/ul>\n<p>However, these scary headlines missed an important point in Blahous\u2019 study: In terms of total (federal, state and private) spending on healthcare, Sanders\u2019 Medicare for All plan is actually projected to cost $2.1 trillion <i>less<\/i> than projections of spending under the current US healthcare system.<\/p>\n<p>Yet a cursory glance at Blahous\u2019 study would not show this immediately. Blahous obscured his own findings by not displaying the ten-year totals for Medicare for All cost savings in his tables. As Matt Bruenig, head of the People\u2019s Policy Project think tank, pointed out in <b>Jacobin<\/b> (<a href=\"https:\/\/jacobinmag.com\/2018\/07\/medicare-for-all-mercatus-center-report\">7\/30\/18<\/a>), adding up all the values for Medicare for All cost savings from 2022\u201331 in <a href=\"https:\/\/www.mercatus.org\/system\/files\/blahous-costs-medicare-mercatus-working-paper-v1_1.pdf\">Table 2<\/a> of Blahous\u2019 study shows that enacting Medicare for All would actually save $2.1 trillion in national health expenditures over that period. It\u2019s as if Blahous saw the results of his data and didn\u2019t like them, so he just decided to not publicize them in his table.<\/p>\n<p>While the <b>AP<\/b> story (<a href=\"https:\/\/www.apnews.com\/09e06d686a1a481fa76e3fd91f3fcbc2\/Study:-'Medicare-for-all'-bill-estimated-at-$32.6-trillion\">7\/30\/18<\/a>) did acknowledge the cost savings (with caveats on whether doctors and hospitals would accept rates commensurate to that of Medicare on a larger scale), most of the articles did not address Medicare for All cost-saving explicitly. The article by <b>Axios<\/b> (<a href=\"https:\/\/www.axios.com\/medicare-for-all-bernie-sanders-federal-cost-33-trillion-study-23251fc1-1888-4d4b-964d-21cacf703bf1.html\">7\/30\/18<\/a>) buries this same lede in the bottom of its \u201cworth noting\u201d section:<\/p>\n<p style=\"padding-left: 30px\">All told, &#8220;Medicare for All&#8221; would actually slightly reduce the total amount we pay for healthcare. But the plan would increase the share of that cost paid through taxes, rather than through insurance premiums or out of pocket<\/p>\n<p>Blahous\u2019 omission makes sense, given the small-government, market-centered political ideology of the Mercatus Center, which is funded primarily by conservative billionaires Charles and David Koch, who have given <a href=\"http:\/\/conservativetransparency.org\/recipient\/mercatus-center\/?og_tot=289&amp;order_by=&amp;adv=koch&amp;min=&amp;max=&amp;yr=&amp;yr1=&amp;yr2=&amp;submit=\">at least<\/a> $9 million to the think tank and <a href=\"https:\/\/www.newyorker.com\/magazine\/2010\/08\/30\/covert-operations\">tens of millions<\/a> to George Mason University. The organization was originally founded in 1980 by Richard Fink, a former executive vice president of Koch Industries, and both he and Charles Koch currently <a href=\"https:\/\/www.mercatus.org\/board\">sit<\/a> on its board of directors. Blahous, also a member of the Walton and Scaife family-<a href=\"https:\/\/www.insidephilanthropy.com\/home\/2015\/4\/21\/how-the-hoover-institution-vacuums-up-big-conservative-bucks.html\">funded<\/a> Hoover Institution at Stanford University, has long <a href=\"https:\/\/www.hoover.org\/profiles\/charles-blahous\">warned<\/a> about the supposed fiscal strain of Social Security and other \u201centitlement\u201d programs, though at other times he\u2019s seemed to <a href=\"https:\/\/www.huffingtonpost.com\/2012\/12\/12\/social-security-fact-checkers-fiscal-cliff_n_2286462.html\">contradict<\/a> his dire appraisal.<\/p>\n<div id=\"attachment_8927412\" style=\"width: 360px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-8927412 size-full\" src=\"https:\/\/fair.org\/wp-content\/uploads\/2018\/07\/Screen-Shot-2018-07-31-at-4.41.05-PM-e1533070370690.png\" alt=\"Slate: Conservative Think-Tanker Accidentally Argues That Single Payer Could Save Americans $2 Trillion\" width=\"350\" height=\"390\" \/><\/p>\n<p class=\"wp-caption-text\"><em><strong>Slate<\/strong> (<a href=\"https:\/\/slate.com\/business\/2018\/07\/single-payer-health-care-could-save-americans-usd2-trillion-conservative-think-tanker-accidentally-argues.html\">7\/30\/18<\/a>)<\/em><\/p>\n<\/div>\n<p>Sanders\u2019 Medicare for All plan would replace the current system of private health insurers, which has seen healthcare cost continuing to rise <a href=\"http:\/\/fortune.com\/2018\/02\/15\/healthcare-prices\/\">faster<\/a> than inflation even after the passage of the Affordable Care Act in 2010. (Drug prices in particular have <a href=\"https:\/\/www.newsweek.com\/2018\/05\/04\/medication-keeps-getting-more-expensive-and-big-pharma-wont-explain-why-897925.html\">risen<\/a> by 12 percent annually over the past five years.) According to the federal Center for Medicaid and Medicare Services (CMS), total healthcare expenditures were almost <a href=\"https:\/\/www.cms.gov\/Research-Statistics-Data-and-Systems\/Statistics-Trends-and-Reports\/NationalHealthExpendData\/NationalHealthAccountsProjected.html\">$3.5 trillion<\/a> in 2017, or about <a href=\"https:\/\/www.cms.gov\/Research-Statistics-Data-and-Systems\/Statistics-Trends-and-Reports\/NationalHealthExpendData\/NationalHealthAccountsProjected.html\">18 percent<\/a> of US GDP. These figures are by far the <a href=\"https:\/\/interactives.commonwealthfund.org\/2017\/july\/mirror-mirror\/\">highest in the world<\/a>, in both dollar terms and percentage of GDP. And it\u2019s getting worse: The CMS <a href=\"https:\/\/www.cms.gov\/Research-Statistics-Data-and-Systems\/Statistics-Trends-and-Reports\/NationalHealthExpendData\/Downloads\/ForecastSummary.pdf\">estimates<\/a> that costs will increase an average of 5.5 percent each year, and that by 2026, total national health expenditure will amount to $5.7 trillion annually, almost 20 percent of US GDP.<\/p>\n<p>Yet for all its spending on healthcare, the US <a href=\"https:\/\/data.oecd.org\/healthstat\/life-expectancy-at-birth.htm\">lags behind<\/a> the rest of the developed world in life expectancy, which <a href=\"https:\/\/www.cnn.com\/2017\/12\/21\/health\/us-life-expectancy-study\/index.html\">fell<\/a> over the past two years for the first time since 1963. Forty million Americans are still <a href=\"https:\/\/www.reuters.com\/article\/us-usa-healthcare-uninsured\/u-s-healthcare-uninsured-rises-most-in-near-decade-gallup-idUSKBN1F523O\">uninsured<\/a>, which has been shown to <a href=\"http:\/\/annals.org\/aim\/fullarticle\/2635326\/relationship-health-insurance-mortality-lack-insurance-deadly\">increase<\/a> <a href=\"http:\/\/www.pnhp.org\/excessdeaths\/health-insurance-and-mortality-in-US-adults.pdf\">mortality rates<\/a>. According to a 2015 Kaiser Family Foundation\/<b>New York Times<\/b> <a href=\"https:\/\/www.kff.org\/report-section\/the-burden-of-medical-debt-section-3-consequences-of-medical-bill-problems\/\">survey<\/a>, 26 percent of Americans ages 18\u201364 reported having trouble paying their medical bills, 2 percent of which declared bankruptcy.<\/p>\n<p>Sanders\u2019 Medicare for All plan aims to remedy cost issues through massive administrative cost reductions by cutting out health insurance middlemen and savings from applying Medicare cost rates, which are significantly lower than private insurance rates due to the strength of government bargaining power on costs for care. Of course, it also plans to provide guaranteed coverage to every American.<\/p>\n<p>But if Sanders\u2019 plan actually amounts to net savings on overall healthcare spending, why do the Mercatus Center and all of the publications that cite it make such a fuss over the seemingly giant and scary multi-trillion dollar price tag of Medicare For All even when it\u2019s fiscally sound? The answer is of course, the big conservative boogeyman: taxes.<\/p>\n<p>Since Medicare for All essentially shifts payment for healthcare from premiums, deductibles and co-pays to taxes, anti-tax crusaders like the Kochs and their minions at the Mercatus Center would naturally be opposed. In the <b>AP<\/b> article, Emory professor and former Clinton administration adviser Kenneth Thorpe notes pointedly, \u201cThere are going to be a lot of people who\u2019ll pay more in taxes than they save on premiums.\u201d Those people will certainly be at the top end of the income scale.<\/p>\n<p>The media chorus complaining about how much a program like Medicare for All adds to the federal debt fades to a whimper, however, when it comes to enacting <a href=\"https:\/\/www.politico.com\/story\/2018\/02\/28\/tax-cuts-trump-gop-analysis-430781\">trillion-dollar<\/a> <a href=\"http:\/\/www.latimes.com\/politics\/la-na-pol-tax-cuts-debt-20180626-story.html\">tax cuts<\/a> for the <a href=\"http:\/\/www.chicagotribune.com\/news\/nationworld\/ct-analysis-tax-cut-deficit-20180409-story.html\">rich<\/a>, providing <a href=\"https:\/\/www.google.com\/search?biw=1346&amp;bih=619&amp;ei=v9FfW_C2JaSHggfBl4PADw&amp;q=corporate+welfare+2017+amazon&amp;oq=corporate+welfare+2017+amazon&amp;gs_l=psy-ab.3...20405.21177.0.21396.7.7.0.0.0.0.166.693.5j2.7.0....0...1.1.64.psy-ab..0.6.526...0i22i30k1j33i22i29i30k1.0.X0b5PyoHOJk\">billions<\/a> in <a href=\"https:\/\/www.cnbc.com\/2017\/07\/11\/states-pay-as-much-as-2-million-per-job-to-attract-tech-companies.html\">corporate<\/a> <a href=\"https:\/\/newrepublic.com\/article\/146540\/amazon-thriving-thanks-taxpayer-dollars\">welfare<\/a>, or fighting <a href=\"https:\/\/www.newsweek.com\/how-many-trillions-war-has-cost-us-taxpayer-911-attacks-705041\">endless<\/a> and <a href=\"https:\/\/www.cnbc.com\/2014\/07\/31\/how-dods-15-trillion-f-35-broke-the-air-force.html\">fruitless<\/a> <a href=\"https:\/\/www.reuters.com\/article\/us-iraq-war-anniversary-idUSBRE92D0PG20130314\">overseas<\/a> <a href=\"https:\/\/money.cnn.com\/2017\/08\/21\/news\/economy\/war-costs-afghanistan\/index.html\">wars<\/a> with America\u2019s $700 billion\u2013a\u2013year military <a href=\"https:\/\/www.defensenews.com\/breaking-news\/2018\/02\/07\/congress-reaches-budget-deal-with-huge-defense-boost\/\">budget<\/a>.<\/p>\n<p>Some publications, largely but not entirely progressive ones, like <b>Jacobin<\/b> (<a href=\"https:\/\/jacobinmag.com\/2018\/07\/medicare-for-all-mercatus-center-report\">7\/30\/18<\/a>), <b>Vox<\/b> (<a href=\"https:\/\/www.vox.com\/policy-and-politics\/2018\/7\/30\/17631240\/medicare-for-all-bernie-sanders-32-trillion-cost-voxcare\">7\/30\/18<\/a>), <b>Common Dreams<\/b> (<a href=\"https:\/\/www.commondreams.org\/news\/2018\/07\/30\/koch-funded-hit-piece-backfires-shows-medicare-all-would-save-300-billion-over-ten\">7\/30\/18<\/a>), <b>Business Insider<\/b> (<a href=\"https:\/\/www.businessinsider.com\/bernie-sanders-medicare-for-all-plan-cost-save-money-2018-7\">7\/30\/18<\/a>), the <b>New Republic<\/b> (<a href=\"https:\/\/newrepublic.com\/minutes\/150286\/new-study-universal-health-care-shows-partisans-bad-reading-studies\">7\/30\/18<\/a>), the <b>Week<\/b> (<a href=\"http:\/\/theweek.com\/speedreads\/787724\/bernie-sanders-thanks-koch-brothers-accidentally-making-case-medicare-all\">7\/30\/18<\/a>), <b>Slate<\/b> (<a href=\"https:\/\/slate.com\/business\/2018\/07\/single-payer-health-care-could-save-americans-usd2-trillion-conservative-think-tanker-accidentally-argues.html\">7\/30\/18<\/a>) and <b>Vice<\/b> (<a href=\"https:\/\/www.vice.com\/en_us\/article\/594ynd\/bernies-medicare-for-all-might-cost-dollar326-trillion-and-thats-fine\">7\/30\/18<\/a>), did not naively parrot the Mercatus study\u2019s big price tag. These outlets pointed out the large cost savings of Medicare for All implicit in the study, and the benefits of universal healthcare coverage from both an ethical and a cost perspective.
